WHS Physical & Immunization Requirements - Attention Grade 10 parents/guardians

Attention Grade 10 parents/guardians: In accordance with MA Department of Public Health mandates, all students entering grade 11 must have the following BEFORE entry to 11th grade:

1) Physical Examination
2) Up-to-Date Immunizations - including Meningococcal Vaccine

Your child's pediatrician has a comprehensive knowledge of the health status of your child, so please make an appointment to have him/her perform the school requirements. Immunization records of all incoming grade 11 students are due PRIOR to entry to grade 11. The Meningococcal vaccine, (formerly MCV4), must be given on or after the 16th birthday and ≥ 8 weeks after the previous dose. 1 dose is acceptable if it was given on or after the 16th birthday. Meningococcal B vaccine is not required and does not meet this requirement. To enable the process of these records, please submit the paperwork no later than June 1st. The school physical exam form may be found on the Health Documents and Forms page in the left menu. Please plan accordingly, as school exclusion mandates are applied to non-compliant students.

You may fax the information directly to the nurse's office, submit by mail or have your child hand deliver to the School Nurse.
